Send your enrolment form (with both sides completed) with your credit card details, cheque or money order made payable to the Deaf Society of NSW.

Credit card enrolments only. 24 hours ~ 7 days. Fax both sides of your completed enrolment application form giving credit card details including the type of credit card, expiry date, card holder’s name and signature. Please note that there is NO extra Enrolment Fee for enrolments submitted by fax. Please do not enlarge or reduce the size of form.

Bring both pages of your completed enrolment form and your cheque, cash, money order or credit card to our office. Enrolments are received between 9am and 4pm, Monday to Friday.
